Safety culture problems continue in Olkiluoto

STUK has released their 2009 Q3 report. Still problems with welding of
steel liner – this time welds being grinded too thin and the defect
passing three layers of quality inspections before STUK noticed it.

A steam generator pipe was cut in the wrong place after pressure test.
Had to be fixed and test had to be repeated.

Primary coolant pump seals did not work as expected, testing continues.

Still problems with training workers on safety culture, lack of
communication from workers to quality controllers.

Installation guidelines not in conformance with requirements. TVO has
not shown that production weld tests done according to regulation.
